Default Smoke in WW II

Given the extremely useful nature of smoke in the game, I was wondering if anyone has heard of any battles, skirmishes, etc. where smoke was used in combat situations. I can only recall three examples offhand:

1) Cover for Major Cook's Waal River assault during Market Garden (the smokescreen blew away by most accounts).
2) Cover for engineers removing anti-tank mines during the first stage of Montgomery's El Alamein offensive.
3) Cover for tanks--fired by the tanks themselves--of the Sherwood Rangers when ambushed by Tigers .

So why wasn't it used more often? Or was it? I've never heard of smoke used to any significant degree in major landings like D-Day or Tarawa. Just wondering aloud why not; seems like it would prevent many casualties if properly employed under the right conditions.

You can see the answer with respect to landings in game, ships produced their own smoke screen when needed so navel units do not have smoke ammo.
As far as I know & dont take it as right USA & Brits used smoke quite a lot but it was in short supply for other nations

It is my understanding that when assaulting pillboxs and other fixed positions it was SOP for German Pioneer unite to use smoke to cover there approach.

WP grenades and mortars were used which produce smoke.
