Only Jotuns turn into giant werewolves. There is no size check for the amulet. In fact, if you give the amulet to a giant, they may turn into a normal werewolf.

Unless your commander has blood levels, if you're just looking to bump the unrest as much as possible you're probably better off pillaging (which will net you some extra gold vs. the blood slaves.)

In CBM 1.7 (haven't checked 1.8) it does AOE 1 damage like a fire/frost brand (but -15 damage). So, on a very large, very strong unit (e.g. an ettin) it can be quite effective.
