I just tested Manifestation.
AdmiralZhao, MaxWilson, and Gregstrom are correct.
Either of one of your own army commanders or a besieged army commander can be attacked by the Ashen Angel, it appears to be random (or possibly by unit ID# order). There is no guarantee that a Horror Marked commander will be targeted first by the spell.
It does attack friendly commanders in the target province as well.

Manifestation is probably an "anonymous" spell like Send Horror, Ghost Riders and so on.

Yeah, but so is Infernal Disease, and it targets the enemy only.

Yes, I've tested infernal disease for two rounds X2. All four shots hit enemy commands inside fort. It seems it works according to the description.
