What does it really do and is it worth the 150 blood slaves?

There is a possibility that a unit under an enemy commanders command in your dominion will attempt to assasinate its commander under the command of a devil.
I think higher dominion and lower morale of the unit increase chance of succes.

Is the test only made for one unit per commander, or might multiple units turn on their commander?

I think only for one unit per commander, because I haven't seen multiple assasinations on the same commander in a turn. Also I haven't seen undead, demons or mindless units betray their commander.

No idea.
The assasinations seem to happen often with 10 morale units. If your enemy has a lot of normal units in your dominion it might be worth it.

As a bonus (well, haven't checked recently, so this might have changed) -- if the soldier tries but dies, the devil (who otherwise stands and watches) takes a stab at it.
Hmmmm, possible bug -- I seem to recall it working underwater. Devils aren't amphibious, are they?
