What he means is to decrease the hardware acceleration of sounds. Its in Control Panel -> Sounds and Audio Devices -> Advanced Button -> Hardware Tab.

As a clarification to others having the same problem:
I have an OEM SoundbLaster Live Value. I had the same access violation message. I downloaded and installed the latest Unified driver release from www.soundbLaster.com and everything works perfectly. I fthis is your issue, then this may be your answer.

Had the same problem, it would crash after name selection and map creation. After several hours swearing and downloading drivers, it works fine now.
Problem appears to have been the SoundBLaster Live! drivers. I updated the GeForce2 display drivers, and still got the problem, so then I updated the SB drivers and it works fine now.
Wierd thing is: Demo ran fine on the same rig with the old drivers *shrug*
